A medical record review summary can read cleanly and still be wrong in a 2025 UCSF study. Physicians who reviewed AI-written hospital discharge summaries against the underlying notes found nearly three errors per summary.
For a workers’ compensation attorney, that gap matters more and those errors carry a heavy price. Every fact in a medical summary eventually has to be backed by a reference from the record itself, and source linkage is what makes that proof legit.
What source linkage means in medical record review?
A workers’ compensation file rarely arrives in order. It comes as thousand of scanned pages from treating physicians, urgent care clinics, chiropractor reviews with reports highlighting the series of health issues and treatment undergone by the affected Before anything can be summarized, a record review platform makes every page machine-readable,removes duplicates, puts the record in date order and stamps each page with a unique identifier number.
Source linkage builds on that page address. Each entry in the summary carries a link to the stamped page it came from, so clicking a line about 4 year back lumbar strain opens the urgent care note that recorded it.
That is different from a citing an entire report. A note that simply names a progress report and its date sends the reader to a document that may run thirty pages, and the hunt starts from there. A page link removes the search for data, which matters more when a petition is days away and every fact needs a cite.
The difference sounds small until you see how often a polished summary gets the record wrong.
A clean summary is not proof
The
UCSF study, published in JAMA Internal Medicine compared AI-written and physician-written discharge narratives for 100 hospital stays. Twenty-two attending physicians reviewed the narratives alongside the physician-written versions, using the full set of notes as the reference. The AI’s versions averaged 2.91 errors each, compared with 1.82 for the physicians’, with more omissions and inaccuracies. They were also rated as more concise and coherent.
That combination is the problem. A summary that reads smoothly gives no hint of what it got wrong, and the reviewers could only find the errors because they had the source notes in front of them. A page link puts the source in front of every reader, so checking a line becomes a click instead of a search.
Why the WCAB makes every page count
Under
WCAB Rule 10945, every petition for reconsideration, removal, or disqualification must fairly state all material evidence on the point at issue and support evidentiary statements with specific references to the record. For a medical report, that means identifying the exhibit number, author, date, and relevant page.
Rule 10972 adds the consequence: a petition unsupported by specific references to the record may be denied or dismissed.
An attorney usually meets this rule against the clock. A working summary without page links has to rebuild those references by hand, usually against a filing deadline. Without page links, every fact drawn from the medical record means going back to the file, finding the page, matching it to the exhibit and writing out the cite.Source linkage removes the slowest step. The page behind each fact is already found, so turning it into the exhibit, author, date and page cite the rule asks for becomes a lookup, not a search.
How one omitted page sank a defense petition
Buendia v. Hospitality Staffing Solutions (WCAB, April 2025) turned on whether an injured worker’s kidney disease and related conditions resulted from anti-inflammatory drugs prescribed for her accepted knee and back injuries. The trial judge concluded that they did, and the decision illustrates how that finding was supported by the record. It relies on the internal medicine QME’s report, citing the exhibit number and page 99, where the evaluator linked the kidney failure to chronic use of prescribed NSAIDs. The decision’s other findings are similarly supported by specific exhibit and page references.
For applicant counsel, that is the job in miniature: when treatment started, what each provider found, which medications were prescribed and why, and where each of those facts sits in the record. Source linkage keeps each of them a click away, so a petition or an answer can carry its cites from the first draft, and a disputed fact can be met with the page that settles it.
The same case shows the other side of the rule.
What source linkage gives defense attorneys?
In Buendia, the defense petition argued there was no evidence that the anti-inflammatory medication had ever been authorized. Its own Exhibit D, a utilization review decision, certified the naproxen request on its first page. The petition never mentioned it. The WCAB ruled that leaving out that exhibit violated Rule 10945, denied the petition and admonished the defendants and their attorneys by name, with a warning that further violations could bring sanctions. The trial judge had recommended sanctions outright.
Defense counsel’s job is to test the claim: earlier injuries to the body part now claimed, gaps in treatment, histories that change from one provider to the next. Rule 10945 also requires stating the material evidence fairly, including the pages that cut against the defense. A linked summary puts the unfavorable record in view along with the favorable, so neither surprises anyone after the petition is filed.
Either way, a link tells you where a fact lives. It doesn’t tell you the fact is right.
How WHITE AI links every entry
WHITE AI, Rapid Care’s medical record review platform for workers’ compensation, builds source linkage into every file:
Organize the record. It splits continuous scans into separate documents, removes duplicate copies sent by different sources, and puts every record in date order.
Give every page an address. Each page is stamped with a Bates number and page label, and the set is bound into one hyperlinked PDF with a bookmarked table of contents.
Separate the perspectives. Treating physician, utilization review, QME, and IME records are kept distinct, so a UR certification does not disappear inside a treatment history.
Link the summary. With Sort & Summarize, the medical chronology is written for your role, and every entry carries a hyperlink to the Bates-numbered page it came from.
Check before delivery. Sections the system is less confident about go to human quality control, and a clinician signs off on the summary before it reaches you.
For urgent cases, the turnaround days can be the same day when the page limit allows it. The summary template is configured once and applied to every file after that. If you only need the organized record, sort only delivers the bound, bates-stamped and hyperlinked set without a written summary. Either way, your attorneys and experts still decide what the record means.

Upload a recent case to WHITE AI, open the summary and click any line. If it opens to the right page, you have seen what changes at petition time.
